Septoplasty Without Tampon

A deviated septum occurs when this wall is displaced or bent, which can obstruct normal nasal breathing and lead to various issues. The treatment options for a deviated septum include medications, surgery, and other therapies. The best treatment depends on the severity of the deviation and the symptoms it causes. At Poliklinika Sinteza, we leverage our 15 years of experience in surgical treatment of this condition. With a skilled team and state-of-the-art equipment, recovery is typically very quick, and nasal packing is only rarely required.

What is a Deviated Septum?

A deviated septum can cause difficulty breathing through the nose, leading to headaches, fatigue, reduced sense of smell, and altered resonance (nasal voice). The condition can lead to mouth breathing, which exposes the throat, larynx, and trachea to cold and dry air, resulting in chronic inflammation, painful swallowing, chronic tonsillitis, frequent sore throats, or chronic inflammation of the larynx and trachea, often accompanied by a persistent cough.

In children, the improper airflow through the Eustachian tube can cause frequent ear infections and potential hearing loss. Sleep disturbances, such as snoring and waking up tired, are also common. Persistent nasal discharge after a cold can take a long time to normalize.

Among the most common causes of nasal obstruction are deviations of the nasal septum and hypertrophy (enlargement) of the nasal turbinates. Deviations can be congenital or the result of injury at any age. The deviation mechanically narrows one or both nasal passages, reducing airflow. Nasal sprays are ineffective for this issue because they only address the mucosa, not the cartilage or bone. Nasal sprays are ineffective for this issue because they only address the mucosa, not the cartilage or bone.

What is Septoplasty?

Septoplasty, commonly known as nasal septum surgery, corrects a deviated septum, which is the wall separating the nasal passages. This surgery is performed through one nostril without any external incisions. The curved cartilage is partially removed, and the remaining structure is straightened. Modern techniques such as R-Frequency and functional endoscopic nasal surgery minimize tissue trauma and typically do not require nasal packing.

For chronic nasal turbinate enlargement, laser or R-Frequency reduction methods are preferred. Our extensive experience confirms their effectiveness in mild to moderate cases. For severe turbinate hypertrophy, especially involving the bony structures, surgical intervention is recommended.

Who Needs Septoplasty?

Septoplasty is usually recommended for individuals with significant breathing difficulties due to a deviated septum.

The surgery may be advised if:

  • Breathing problems are caused by severe septal deviation not responsive to conservative treatments like allergy medications or nasal sprays.
  • The patient experiences snoring or breathing difficulties during sleep, which surgery might help alleviate.

Not all cases of septal deviation require surgery; a decision should be made after a thorough examination by an ENT specialist who will assess the condition and recommend the best treatment option.

How to Prepare for the Procedure?

Preparation for septoplasty may vary based on specific requirements of the procedure. Here are general tips to help you prepare:

  1. Discuss Expectations: Talk to your doctor about what to expect during and after the procedure.
  2. Check Medications: Review your current medications with your doctor to understand if you need to adjust them before the surgery.
  3. Limit Food and Drink: Depending on the anesthesia used, you may need to restrict food and drink intake before the procedure. Your doctor will provide specific instructions.
  4. Arrange Transportation: You will not be able to drive home after the surgery, so arrange for someone to take you home.
  5. Prepare for Recovery: Follow your doctor’s instructions for post-operative care, including managing pain, limiting physical activity, and caring for the surgical site.
  6. Arrive on Time: Arrive at the clinic at the scheduled time for your procedure.

These are general guidelines, so make sure to follow any specific instructions given by your doctor.

Before deciding on septoplasty, a thorough diagnostic evaluation is conducted, including:

  • Rhinoscopy
  • Nasal endoscopy
  • Fiberendoscopy
  • Measurement of nasal airflow (rhinomanometry)

Septoplasty is performed under short-term anesthesia, which requires:

  • Evaluation by our anesthesiologist
  • Laboratory tests
  • ECG
  • Spirometry

Modern pain relief methods ensure minimal discomfort after the surgery.

Recovery

Our extensive experience, combined with precise surgical techniques, allows for septoplasty and turbinate surgeries to be performed as day surgeries, meaning patients can return home a few hours after the procedure.

There are usually no visible changes such as swelling or bruising. Nasal packing is rarely needed after septoplasty. For turbinate surgeries, packing remains in the nose for only a few hours, at most overnight. In the days following the surgery, the nasal mucosa may be swollen, and nasal drops and emulsions are used to reduce edema and accelerate recovery.

Recovery from septoplasty can vary from patient to patient but generally lasts from a few days to a few weeks. You can return to daily activities after 3-4 days and resume physical activities after 3 weeks. Here are some typical recovery steps:

1. Elevate Head: Sleep with your head elevated for the first few days to reduce swelling and prevent bleeding.

2. Avoid Physical Activity: Avoid strenuous activities and physical exertion during the first week to prevent pressure on the nose.

3. Use Cold Compresses: Applying cold compresses to the nose can help reduce swelling.

4. Take Prescribed Medications: Regularly take prescribed medications to manage pain and reduce swelling.

5. Avoid Smoking: Smoking can delay healing, so it’s best to avoid smoking during recovery.

6. Follow-up Appointments: Adhere to follow-up visits with your doctor to monitor healing and recovery.

7. Avoid Spicy Foods: For the first few days, avoid spicy or hot foods that may irritate the nose and throat.

Every patient’s recovery is unique, so follow your doctor’s specific instructions for post-operative care.
