Treatment of Lipedema

Lipedema is a congenital disorder characterized by abnormal distribution of subcutaneous fat primarily in the hips and legs. Often, the arms are also affected, while the feet and hands remain spared from edema. Lipedema almost exclusively occurs in women, suggesting hormonal causes, and typically manifests during puberty or pregnancy.

Lipedema is not fat tissue resulting from obesity but rather fat cells that have undergone pathological changes. Therefore, diet alone cannot address lipedema, although physical activity and weight control indirectly have a positive effect on the condition.

Lipedema develops symmetrically in both extremities. Initially, changes occur in the hips and buttocks, presenting as “riding pants” (Reiterhosen, Turkish pants) and cellulite. Over time, the thighs and eventually the calves (Suavenhosen-Phänomen) become involved. The ankles and feet are typically free from edema. Lipedema affecting the inner thighs and knees can impair walking and potentially lead to skin damage in these areas.

The only treatment for lipedema that allows for permanent removal of the condition is gentle vibroliposuction under local tumescent anesthesia. The procedure is performed multiple times depending on the degree and extent of the lipedema. Conservative treatment of lipedema through manual or device-assisted lymphatic drainage and the use of compression stockings precedes and continues after liposuction, providing important support over an extended period.
